HOSPICE MEDICAL CARE

Integrity Hospice Care services are provided in the comfort of your home, assisted living facility, hospital or nursing facility. Our individualized care plans ensure that you and your loved one receive the support and compassionate care you need.

EXPERIENCED CARE TEAM

Integrity provides a team-based approach to care. This team includes a team of physicians, nurses, hospice aides, social workers, chaplains, bereavement counselors and trained volunteers. Our Hospice Care Teams, which meet weekly to review care plans, are coordinated by a Patient Care Manager. They work together to ensure that the patient’s physical, emotional and spiritual needs are always met.

EMERGENCY-READY

Whatever the time of the day or the day of the week it is, we’re here to help! We provide the assistance and support for the patient and family on a 24-hour, seven days per week basis. Patients are provided with routinely scheduled in-home visits by members of our hospice team.

AFFORDABLE HELP

Hospice services are covered under Medicare Part A, Medicaid and most private insurances. This benefit covers the Hospice Care Team’s services, medications, supplies and equipment related to the patient’s life-limiting illness. There may be co-pays, co-insurance or a deductible under insurance plans.
